Job Description

Woodrow Mercer have partnered with an NHS organisation in the West Midlands to source an RMN CHC Nurse Assessor and an RGN CHC Nurse Assessor for a 3 month contract outside IR35 paying up to £350 per day.

RGN Duties inlcude

  • Liaising with CCG CHC team, CSU CCG team, provider services,
  • Carrying out DSTs for patients where it has been identified there may be a change in need
  • Liaising with the wider multidisciplinary team to provide advice and support regarding CHC and ensuring patients are moving via the correct discharge pathways
  • Carrying out DSTs for patients that have been funded for an extra period of assessment in a timely manner
  • Support EAB colleagues with advice and support regarding the CHC framework
  • Tackle delays in EAB beds where this is related to CHC
  • Support our data cleansing team with helping to transact any changes that will make savings for the CCG
  • Renegotiate price for packages of care where we have identified that we are paying too much or the patients' needs have changed
  • Carry out DSTs for fast track reviews where these are delayed
  • Providing professional challenge where appropriate if there are inappropriate referrals

RGN must have substantial experience continuing health care assessments and commissioning care packages for CHC

RMN duties include

  • Liaising with CCG CHC team, CSU CCG team, provider services,
  • Carrying out DSTs for patients with a functional mental health background and patients with a mental health background who have a history of challenging behaviours, liaising with the multi-disciplinary team to give advice and support regarding CHC and ensuring patients are moving via the correct discharge pathways
  • Providing professional challenge where appropriate if there are inappropriate referrals
  • Assessing patients who are being funded by the CCG for 1:1 supervision - ensuring that other least restrictive principles are being considered and advising on other strategies that could be used to manage challenging behaviours
  • Managing/overseeing admission into and discharge out of 7 challenging behaviour beds that the CCG has recently commissioned

RMN must have substantial experience in functional mental health, continuing health care assessments, assessing patients with challenging behaviours and assessing the need for one to one nursing. This role will be working with patients who have a mental health background and usually have more challenging behaviours.
