Job Title: Primary Care Network Care Coordinator

Contract Type: 6 months fixed term

Hours: Flexible, see below 

Salary: £25K  

Location: North Kensington, remote, home-based working      

This is an exciting and unique opportunity to be part of making history! This job is to help with the roll out of the COVID-19 vaccination across North Kensington and we are looking for hardworking, motivated people to come on board.

This role is working for a ‘Primary Care Network’ (PCN), which is a group of GP clinics who work together, in this case focusing on delivering the COVID-19 vaccination across the locality.

As a Care Coordinator you will:

  • Contact patients to book their vaccination appointment
  • Advise patients on the safety of the vaccination and answer any questions and concerns they may have (you will be provided with a script and all of the medical information that you’ll need to do this)
  • Follow up with patients 2 days before their appointment to remind them to attend
  • Using ‘emis Systm one’ to access and update patient records (training on this is available).
  • Working and liaising with a multidisciplinary team

Mode of work:

  1. This is initially a 6-month fixed term contract, during which time the work will be remote, home based.
  2. After the fixed term contract there is the opportunity to join on a permanent basis, at which point you would need to travel into the North Kensington office on occasion.
  3. Standard working hours are 9-5pm but the clinic is open from 8am – 8pm so there is the flexibility to start / finish earlier or later
  4. Option to work 4 or 5 days per week
  5. Laptop and phone provided

What we need from you:

  • Excellent communication and telephone skills
  • Be comfortable in talking to people from all walks of life and backgrounds
  • Good organizational and time management skills
  • Experience in a similar healthcare / administration position is desirable
  • Ideally be proficient in using emis System One
  • Be a self-starter and able to stay motivated and focused whilst working from home
