Job Title: Primary Care Network Care Coordinator – Diabetes and long term conditions

Contract Type: 6 months fixed term

Hours: 37.5 per week with opportunity for paid over-time 

Salary: £25K  

Location: North Kensington Medical Centre, W10 6NX

This role is working for a ‘Primary Care Network’ (PCN) called Neohealth. A PCN is a group of GP clinics who work together delivering various areas of primary care. Neohealth play an important role in their community, supporting people from all kinds of different backgrounds and are like one big family, which you will join as a Care Coordinator. 

As a Care Coordinator you will: 

  • Contact patients to book their appointments
  • Setting up on-line patient records and supporting with care planning
  • Updating patient records to track progress
  • Planning and facilitating virtual group consultations 
  • Ensuring that appointment availability is maximised across various areas of primary care including: people with long term conditions and people with diabetes 
  • Planning and coordinating appointment slots with availability of GPs
  • Follow up with patients 2 days before their appointment to remind them to attend
  • Using ‘emis Systm one’ to access and update patient records (training on this is available).
  • Working and liaising with a multidisciplinary team

Mode of work:

  1. This is initially a 6-month fixed term contract, during which time the work will be a mix of remote, home based working and office based at the practice when required
  2. After the fixed term contract there is the opportunity to join on a permanent basis and continue to develop your career with the NHS 
  3. 37.5 hours a week with the option to take on paid over-time 
  4. Laptop and phone provided 

What we need from you:

  • Excellent communication and telephone skills
  • Be comfortable in talking to people from all walks of life and backgrounds 
  • Good organisational and time management skills
  • Experience in a similar healthcare / administration position is desirable
  • Ideally be proficient in using emis System One
  • Be a self-starter and take incredibly busy days in your stride!
  • Flexibility with working hours
